Confidentiality
BUSR and FUSR regard all employee information as confidential and will respect the employee’s right to privacy.
As the nature of business of BUSR and FUSR is medical, and therefore confidential in nature, all employees must respect a high level of confidentiality when it comes to information dealt with on a day-to-day basis. As such, employees will be asked to sign and act in accordance with the following statement or another such form of it:
Employees, members of the medical staff, volunteers, other workers or students associated with Cancer Care Ontario (CCO-OBSP), Burlington Ultrasound and Radiology (BUSR), and/or Fairview Ultrasound and Radiology (FUSR) will have access to information and material relating to patients, employees, other individuals, CCO, BUSR, and/or FUSR, which is of a private and confidential nature.
- All workers shall respect the privacy and dignity of patients, employees and all associated individuals.
- All workers shall treat all CCO, BUSR and FUSR administrative, financial, patient, employee and other records as confidential material, and will protect them to ensure full confidentiality. All workers shall not read records or discuss, divulge or disclose such information unless there is a legitimate purpose as it relates to their employment.
- All workers shall ensure that confidential information is not inappropriately accessed, used or released either directly by me, or by virtue of their signature or security access to premises or systems.
- All workers shall only access, process and transmit confidential information using CCO, BUSR or FUSR hardware, software, and other authorized equipment, as required by the duties of their position. All workers understand that CCO, BUSR or FUSR may conduct periodic audits to ensure compliance with policies and to ensure the integrity of information.
- All workers shall only access, process and transmit information using the Internet as required by the duties of their position.
It will be understood that should a worker breach any of these conditions, the worker will be subject to disciplinary action up to and including termination, loss of privileges or similar action. The above list is not exhaustive in regard to confidentiality issues.
The signed confidentiality agreement will be retained in the employee’s file.

PRIVACY STATEMENT
Collection of Exam Preparation
We collect personal health information about you directly from you or from the person acting on your behalf.
The personal health information we collect may include your name, date of birth, address, phone number, health history, records of your visits to Burlington Ultrasound and Radiology and/or Fairview Ultrasound and Radiology and the care you received during those visits.
Occasionally, we collect personal health information about you from other sources if we have obtained your consent to do so or if the law permits.
Uses and Disclosures of Personal Health Information
Access to your personal health information is available to those who need to know to provide care, including: physicians, nurses, technicians, therapists and other health professionals.
We use and disclose your personal health information to:
- treat and care for you,
- ensure the continuity of your care when you are transferred to another facility,
- get payment for your treatment and care (from OHIP, WSIB, your private insurer or others),
- plan, administer and manage our business operations,
- conduct risk management and research activities,
- provide clinical instruction,
- compile statistics,
- comply with legal and regulatory requirements and
- fulfill other purposes permitted or required by law.
Your family physician is considered an important member of the health-care team. If you are receiving care at our clinic, a copy of your diagnostic test results will be forwarded to your family physician unless you request otherwise. Copies of these results may be sent to other physicians, nurses, therapists and other health professionals provided that the instruction to do so has been written on your requisition or provided verbally by you.
Non-clinical information about your visit such as your name, location within the clinic and current progress in regard to your diagnostic test may be released in order to assist family or friends to locate you within our clinic. If you wish to keep your presence at our clinic private, please notify the reception staff when you check in. They will ensure only the members of your health-care team will know you are in clinic receiving care.
Our Safeguards
Maintaining the confidentiality of your information is very important to us.
Burlington Ultrasound and Radiology and Fairview Ultrasound and Radiology have security measures, policies, procedures and technology in place that we review regularly, to ensure that only authorized persons can access your health information.
All Burlington Ultrasound and Radiology and Fairview Ultrasound staff, technologists, nurses, physicians, students and other agents of the clinic must follow the clinic’s privacy and security polices and procedures.
As an employee, member of the medical staff, volunteer, other worker or student associated with Cancer Care Ontario (CCO-OBSP), Burlington Ultrasound and Radiology (BUSR), and/or Fairview Ultrasound and Radiology (FUSR), I will have access to information and material relating to patients, employees, other individuals, CCO, BUSR, and/or FUSR, which is of a private and confidential nature.
- At all times, I shall respect the privacy and dignity of patients, employees and all associated individuals.
- I shall treat all CCO, BUSR and FUSR administrative, financial, patient, employee and other records as confidential material, and I will protect them to ensure full confidentiality. I shall not read records or discuss, divulge or disclose such information unless there is a legitimate purpose as it relates to my employment.

- I shall only access, process and transmit information using the Internet as required by the duties of my position.
I understand and agree to abide by the conditions outlined in this statement. I also understand that should any of these conditions be breached, I will be subject to disciplinary action up to and including termination, loss of privileges or similar action.
